FIBROSE ENDOMYOCARDIQUE: APPORT DIAGNOSTIC DU REHAUSSEMENT TARDIF EN IRM

Nous rapportons lâobservation dâun patient de 40 ans, de type caucasien, ayant une intoxication tabagique  5 paquets /ann©es mais sans notion de s©jour en zone dâend©mie parasitaire ni atopie. Il pr©sentait une asth©nie depuis 12 mois associ©e  une hyper©osinophilie  25.200 /mm3. Huit mois plus tard sont apparus une dyspn©e Stade II fort de la NYHA et des signes de toxicit© multivisc©rale avec syndrome my©loprolif©ratif. lâ©chographie trans-thoracique, il a ©t© observ© une FEVG  60 % avec des signes restrictifs et un thrombus ventriculaire gauche. Au scanner multid©tecteur synchronis©  lâECG, les coronaires ©taient dâaspect normal; une calcification bi-ventriculaire massive et un comblement apical ont ©t© mis en ©vidence. Les s©quences cin©-SSFP en IRM ont montr© une hypokin©sie bi-ventriculaire, une dilatation du massif atrial associ©es  une insuffisance mitrale et une insuffisance tricuspide. Apr¨s injection de Gadolinium, la s©quence de rehaussement tardif a mis en ©vidence une amputation bi-apicale avec r©tention de contraste  pr©dominance sous-endocardique de distribution non vasculaire signant la fibrose et le thrombus.
LâIRM est un examen performant dans le diagnostic non invasif de la fibrose endomyocardique gr¢ce  lâanalyse cin©-morphologique cardiaque. La s©quence de rehaussement tardif en am©liorant la caract©risation du tissu myocardique notamment de la fibrosepermet de diagnostiquer la FEM et envisagerune meilleure priseen charge.
