ASPECTS RADIOCLINIQUES DES PSEUDARTHROSES DU COL DU FEMUR A COTONOU

     RESUME
Objectif: d©crire les aspects radiocliniques, des pseudarthroses du col f©moral rencontr©es au CNHU de Cotonou.
Mat©riel et M©thode : 13 pseudarthroses ont ©t© diagnostiqu©es en 10ans sur 442 cas de fractures du col f©moral. Chez tous les patients, des examens radiographiques du bassin de face compl©t©s par des incidences obliques et de profil de hanches ont ©t© r©alis©es. Tous les clich©s ont ©t© interpr©t©s par un seul et unique m©decin radiologiste.
R©sultats : Tous les patients ©taient de sexe masculin. Ils ©taient ¢g©s de 19  60 ans. Sur les 13 cas de pseudarthrose : 7 ©taient de si¨ge basicervical soit 54%, 3 de si¨ge transcervical (3%) et 3 autres de topographie sous capitale (23%). Les ©carts inter fragmentaires observ©s et mesur©s sur les clich©s de pseudarthrose ©taient de5 mm dans 15% des cas,6 mm dans 46% des cas et4 mm dans 31% des cas et1 mm dans 8% des cas. 2 cas dâost©ite sur pseudarthrose ont ©t© retrouv©s dont une avec n©crose de la tªte f©morale.
Conclusion : Les pseudarthroses encore fr©quentes dans nos conditions de travail constituent une redoutable complication des fractures mal ou non trait©es. En effet, elles sont non seulement invalidantes pour le patient lorsquâelles ne sont pas diagnostiqu©es et trait©es  temps, mais aussi elles constituent une source de difficult©s th©rapeutiques pour lâ©quipe m©dicale qui a la responsabilit© de sa prise en charge.
 ABSTRACT
 Objective: To describe the radiological and clinical features of the pseudarthrosis of the femoral collar in the CNHU of Cotonou.
Material and Method:Â 13 cases of pseudarthrosis were diagnosed in a period of 10 years, among 442 cases of fractures of the femoral collar. All patients underwent conventional x-rays in frontal, laterl and oblique positions of the hip joint. The x-ray films were analyzed by the same radiologist! Results :Â All patients were male; the age range was 19 to 60 years. Among the 13 cases of pseudarthrosis:Â 7 (54%) were basicervical, 3 (23%) were transcervical and 3(23%) were sub-capital. The distances measured between bone pieces varied from 5 mm in 15% of the cases, to6 mm in 46% of the cases and4 mm in 31% of the cases and1 mm in 8% of the cases. 2 cases of osteitis complicating pseudarthrosis were diagnosed with necrosis of the femoral head in one case.Â
Conclusion :  Pseudarthrosis is frequent in our milieu and constitutes a dangerous complication of fractures poorly managed. It also constitutes a source of therapeutic difficulties for the medical team   that has the responsibility of the management.
