LYMPHOMES THORACO-ABDOMINAUX: IMAGERIE ET INDICATION THERAPEUTIQUE A PROPOS DE DEUX CAS CLINIQUES.

But: valuer lâint©rªt de la tomodensitom©trie et de lâ©chographie dans le diagnostic et lâindication th©rapeutique des lymphomes thoraco-abdominaux.
Patients et m©thodes : Il sâagit de la pr©sentation des observations de deux patients r©f©r©s au Service de Chirurgie du CHU de Yaound©.
R©sultats : Dans le premier cas le diagnostic de lymphome malin non hodgkinien avait ©t© retenu par examen dâune biopsie de la masse pectoro-axillaire droite ©voluant depuis huit mois chez une patiente de 54 ans. La tomodensitom©trie avait conduit au diagnostic dâune tumeur avec lyse des c´tes sous-jacentes, une localisation m©diastinale et une localisation spl©nique. Dans le deuxi¨me cas le diagnostic de lymphome lymphoblastique avait ©t© port© chez un enfant de 14 ans r©f©r© pour dysphagie. La TDM avait localis©e une compression deux tiers distaux de lâ?sophage par une masse tumorale si©geant dans le m©diastin moyen, lâespace retrop©riton©al et le hile spl©nique. Une cytoponction ©cho-guid©e trans-gastrique avait permis le diagnostic dâun lymphome lymphoblastique. Dans les deux cas lâextension de la tumeur  la TDM a permis de retenir lâimpossibilit© de tout traitement chirurgical. Les deux patients d©c©d¨rent apr¨s leur premi¨re cure de chimioth©rapie.
Conclusion : La pratique dâune tomodensitom©trie thoraco-abdominale sâimpose devant tout lymphome diagnostiqu©,  la recherche des localisations profondes souvent multiples. Cette ©volution r©duit lâint©rªt de la chirurgie dans le traitement de cette affection.Mots-clés
