Aspects cliniquesetradiographiques de lâarthrose des genoux  Douala (Cameroun).
Résumé
But : D©crire les aspects radiographiques de lâarthrose du genou et d©terminer lâassociation entre les signes cliniques et le stade radiologique observ©.
Patients et M©thode : De juillet  d©cembre 2012, nous avons inclus de fa§on prospective tout patient adulte, venu consulter pour douleurs de lâarticulation du genou ©voluant depuis trois mois au moins et remplissant les crit¨res de d©finition de lâACR de lâarthrose. Les radiographies des genoux ©taient r©alis©es en incidence post©ro ant©rieure de face en charge, de profil et incidence f©moro patellaire puis©taient ©valu©es en utilisant la classification de Kellgren-Lawrence (KL) de lâarthrose.
R©sultats : Sur les 1496 patients vus en consultation externe de Rhumatologie  Douala(864  lâH´pital G©n©ral et 632  lâH´pital Laquintinie),148(9,89%)patients avaient une gonarthrose.Lâ¢ge moyen ©tait de 57 ± 10,6ans. 111 (75%) ©taient des femmes.Soixante dix huit (52,70%) patients avaient une douleur dâintensit© mod©r©e. Lâindice algo fonctionnel de Lequesne moyen ©tait de 8,5 ± 2,9.Lâarthrose touchait les deux genoux dans 104(70,2%) de cas ; 53 (35,8%) de patients pr©sentaient une atteinte radiologique s©v¨re correspondant aux grade III et IV de KL. Les l©sions radiologiques ©taient plus fr©quentes dans les compartiments f©moro-tibial interne (123 ; 83,1%) et f©moro-patellaire (106 ; 71,6%). Il existait une association significative entre lâintensit© de la douleur et le handicap fonctionnel (p<0,01), mais pas entre lâintensit© de la douleur et le stade radiographique (p > 0.05).
Conclusion : Lâarthrose du genou pr©domine dans les compartimentsf©moro-tibial interne et f©moro-patellaire faisant ©voquer un ph©notype propre au Cameroun, avec les l©sions radiologiques s©v¨res chez un tiers des patients  la premi¨re consultation sp©cialis©e. Un diagnostic et une prise en charge pr©coces permettraient dâ©viter lâ©volution vers un traitement chirurgical, encore peu accessible au Cameroun.Mots-clés
