Local reference dose level evaluation in chest radiography in Yaounde

Objectif: Les doses re§ues durant les examens de radiologie conventionnelle ne sont pas pr©cis©ment connues. Le but de cette ©tude ©tait de r©aliser un audit de dose pour les examens du thorax post©rieur-ant©rieur (PA) et lat©ral (LAT), dâanalyser les proc©dures et ©tablir le niveau local de r©f©rence diagnostique (NLRD).
M©thode : Un nombre total de 715 patients pour 882 radiographies issues de sept institutions sanitaires de la ville de Yaound© ont particip©  cette ©tude. La dose  la surface dâentr©e (ESD) des patients adultes effectuant les examens de thorax PA et de thorax LAT a ©t© ©valu©e en utilisant un algorithme math©matique et le facteur de r©trodiffusion standard.
R©sultats: Les ESDs moyennes variaient de 0,28  0,73 mGy pour le thorax PA et de 0,39  2,09 mGy pour le thorax LAT. Pour chaque incidence, il a ©t© not© une grande variation des doses aux patients dans un mªme h´pital et entre les h´pitaux. Le 75¨me percentile de la valeur ESD, 0,39 mGy, de lâexamen du thorax PA est sup©rieure au niveaux de r©f©rence diagnostiques (NRD) obtenus dans des ©tudes similaires dans certains pays dâAfrique et les NRD ©tablis par les organisations internationales. La valeur de 1,33 mGy obtenue  lâexamen du thorax LAT est comparable aux NRD ©tablis par les organisations internationales.
Conclusion : Lâutilisation des courtes distances foyer-film (DFF) et les charges (mAs) ©lev©es ont ©t© identifi©e comme les principales causes de d©passement de dose aux patients. Les autres facteurs ont ©t© lâabsence du control qualit© et la performance des ©quipements.Cette ©tude montre la n©cessit©du respect des protocoles et techniques dâexamens radiologiques, de contr´le qualit© des ©quipements et de lâassurance qualit© permettant de r©duire la dose aux patients dans nos h´pitaux.Â
