Apport de l'©chographie dans la pr©diction de la malignit© du nodule froid thyro¯dien
Résumé
Objectifs: Analyser les ©l©ments pr©dictifs ©chographiques permettant dâ©voquer le caract¨re cytologique b©nin ou malin des nodules thyro¯diens froids.  Â
Patients  et M©thodes: Nous avons ©tudi© 100 nodules froids chez des patients tout venants  la scintigraphie r©alis©e au techn©tium 99 m (99mTC) dans le service de biophysique et m©decine nucl©aire dâAbidjan. Les r©sultats de l'examen ©chographique de ces nodules froids ont ©t© soumis au verdict diagnostique de la cytoponction thyro¯dienne  l'aiguille fine.
R©sultats : La pr©dominance f©minine a ©t© de 90%. L'¢ge moyen ©tait de 44±3 ans avec des extrªmes de 09 et 75 ans. La cytologie a donn© 90% de l©sions b©nignes et 10% de l©sions malignes avec la pr©dominances du carcinome papillaires. Les l©sions b©nignes ©taient compos©s de:63 % de goitres collo¯des, 23% d'ad©nomes v©siculaires, 2% de thyro¯dites, 2% de kystes thyro¯diens. Concernant la topographie, la taille, le nombre, les dimensions nodulaires et certains ©l©ments propres  l'©chographie telle l'©chostructure, calcifications parenchymateuses, le seul ©l©ment discriminant est l'aspect des contours nodulaires. Autrement dit, lorsqu'un nodule froid thyro¯dien pr©sentait des contours irr©guliers  l'©chographie, il pr©sageait  une malignit© cytologique et le contraire  une cytologie b©nigne
Conlusion : Le nodule froid comme les autres l©sions thyro¯diennes est pr©dominant chez la femme. Lâirr©gularit© des contours nodulaires semble li©e  la malignit© nodulaire thyro¯dienne
